Movie events:
In 1954, Marshal "Teddy" Daniels and his new partner Chuck are assigned a task at Ashcliffe Hospital for the mentally ill, criminally accused on an isolated island in Boston Harbor, to investigate the case of the disappearance of Rachel Solando, who was imprisoned for the drowning of her three children, and the only evidence in front of them is a coded note Inconspicuous found in Solando room.

Teddy and his partner arrive before the storm hits the island, and they stay there for a few days waiting for their return to the mainland. Meanwhile, Daniels researches the matter. The hospital's lead psychiatrist, Dr. John Cowley, refuses to hand over the records to them, and they learn that Dr. Lester Sheehan has left the island and has disappeared.

They are then given access to the hospital and searched after procrastination. While interviewing a patient, Daniels writes a secret word in his diary. While there, Daniels feels migraines from the hospital atmosphere, has visions of his involvement in reprisals, and has disturbing dreams about his wife, Dolores. Chanal, who was killed in a fire.
During the investigation, Daniels finds that Solando has suddenly reappeared without any explanation about her whereabouts or her strange disappearance. They do questionable experiments on patients.
Some of them are taken to the lighthouse to be frozen, and Noyes warns Daniels that everyone else is playing an elaborate game tailored to Daniels, including his partner, Dr. Olay.
As Daniels climbs up to a basement where he discovers a woman in hiding who claims to be the real Rachel Solando, he states that she is a psychiatrist in the hospital and that she has discovered experiments with adverse drug reactions in an attempt to develop mind-control techniques.

But before she can report her results to the authorities, she is forced to live as a patient on that island and Daniels returns to the hospital but finds no evidence of this, but Daniels is convinced that Olay took him to the lighthouse on purpose, and here Olay explains to Daniels that he is not an investigator but is In fact, Andrew Ladis, the "most dangerous patient," is imprisoned for killing his wife, Dolores Chanal, after she drowned her children, Edward Daniels and Rachel Solando.

They are a microcosm of Andrew Ladis and Dolores Chanal, and what's more, the little girl in his recurring dreams is his daughter Rachel. Then Daniels wakes up in the hospital under the supervision of Dr. Cowleys and Sheehan, and when he is asked, he tells the truth in a good and coherent way, which satisfies the doctors as evidence of progress in treatment.
But soon Ladis retreats and warns them that it will be his last chance to rid himself of the torment, and after some time Ladis rests on the hospital grounds with Dr. Once again, Dr. Sheehan shook his head.

He then tells Dr. Sheehan that he wants to know if he's a monster or a good man, but he's taken away and the movie ends with a shot of the lighthouse, leaving us in great uncertainty about what Daniels is.
